
Insurance & Payment InformationWe participate with most major insurance companies, please call our office to inquire if we participate/are In-Network with your insurance company. If we are not contracted (considered OUT of network) with your insurance company, or if you have Medicare, we ask for payment in full at the time of service. We encourage all patients to confirm their coverage prior to making an appointment.
Co-payments are due at the time of your appointment along with supplies not covered by insurance companies. We accept cash, checks, MasterCard, Visa, American Express and Discover for your convenience. You will also be responsible for any unpaid balance, once your insurance company has processed your claims.
We are happy to answer any questions regarding your appointments or insurance! Insurance coverage and benefit questions, however, should be directed to your insurance company’s customer service department (number found on your insurance card) or employee benefits/human resources department at your work place.

If you are a first-time patient, please plan on arriving early to complete a patient registration form (or download the Patient Forms from our website and complete them in advance). Please bring the following items to your appointment:
We realize that our patients' time is valuable, and we make every effort to see patients at their scheduled appointment time. We ask that patients provide as much advance notice as possible when running late for an appointment or when canceling. With this advance notice of cancellation, the time can be scheduled for another patient. We have a twenty-four hour cancellation policy. In some instances you may incur a $72 charge for missing your appointment with out notifying our office.
